Aged Care Assessment Teams (ACATs) help older people and their carers decide what kind of care will best meet their needs when they are no longer able to manage at home without assistance. ACATs provide information on suitable care options and can help arrange access or referral to appropriate residential or community care.
This information service gives expert information to help people make the right decision given their financial circumstances. People can access FIS by phone, interview or by attending one of the regular FIS seminars on a wide range of financial and investment topics. The service does not give financial advice.
Commonwealth Respite and Carelink Centres are information centres for older people, people with disabilities and those who provide care and services. Centres provide free and confidential information on community aged care, disability and other support services available locally, interstate or anywhere within Australia. They are funded by the Home and Community Care (HACC) government department.
Community Aged Care Packages (CACPs) are individually planned and coordinated packages of care tailored to help older Australians remain living in their own homes. They are funded by the Australian Government to provide for the complex care needs of older people.
The NSW Department of Community Services (DoCS) is the leading NSW Government agency responsible for community services. They work to promote the safety and wellbeing of children and young people and to build stronger families and communities. They operate a 24 hour telephone helpline.
The NSW Department of Ageing, Disability & Home Care (ADHC) is the leading NSW Government agency responsible for older people, people with a disability and their carers. They also provide home care services to help these people to live independently in their own homes. ADHC has regional offices throughout the state.
Extended Aged Care at Home Dementia (EACHD) packages are individually planned and coordinated packages of care tailored to help older Australians who experience difficulties in their daily life because of behavioural and psychological symptoms associated with their dementia. The packages are to assist them to remain living in their own homes.
